Understanding Neurocritical Care

Coma & Altered Consciousness

Coma is a state of prolonged unconsciousness in which a person cannot be awakened and does not respond to stimuli. Evaluation of coma requires a systematic approach:

  • Structural causes — large strokes, brain hemorrhage, traumatic brain injury, brain tumors, hydrocephalus
  • Metabolic causes — hepatic encephalopathy, uremic encephalopathy, severe electrolyte imbalances, hypoglycemia
  • Infectious causes — meningitis, encephalitis, brain abscess, septic encephalopathy
  • Toxic causes — drug overdose, poisoning, alcohol intoxication
  • Other causes — status epilepticus (non-convulsive), autoimmune encephalitis, posterior reversible encephalopathy syndrome (PRES)

Status Epilepticus

A neurological emergency where seizure activity is prolonged or occurs in rapid succession without recovery:

  • Convulsive status epilepticus — visible seizure activity lasting more than 5 minutes
  • Non-convulsive status epilepticus — subtle or absent external signs; detected on EEG
  • Refractory status epilepticus — not responding to first and second-line treatments
  • Super-refractory status epilepticus — continuing despite anesthetic agents

Raised Intracranial Pressure (ICP)

Elevated pressure within the skull can be life-threatening and requires urgent management:

  • Causes include brain edema, hemorrhage, tumors, hydrocephalus, and venous thrombosis
  • Symptoms include severe headache, vomiting, drowsiness, and vision changes
  • Risk of brain herniation if untreated
  • Medical and sometimes surgical management required

Acute Encephalopathy

Acute diffuse brain dysfunction causing confusion, altered behavior, and decreased consciousness:

  • Septic encephalopathy — brain dysfunction due to systemic infection
  • Hepatic encephalopathy — liver failure affecting brain function
  • Uremic encephalopathy — kidney failure affecting the brain
  • Hypertensive encephalopathy — severely elevated blood pressure
  • Autoimmune encephalitis — immune system attacking the brain

Our Approach to Neurocritical Care

Rapid Assessment

  • Systematic evaluation of consciousness using Glasgow Coma Scale (GCS) and FOUR Score
  • Pupillary examination and brainstem reflex assessment
  • Identification of lateralizing neurological signs
  • Differentiation between structural and metabolic causes

Emergency Investigations

  • CT brain (urgent) — to identify hemorrhage, large strokes, mass lesions, hydrocephalus

Specific Treatment Protocols

  • Stroke protocols — thrombolysis and thrombectomy pathways

Coma Evaluation Protocol
